Supplement Product Liability — Quick Quote
Page 1 of 3What this form covers
This one short form gets you product liability quotes for dietary supplement and nutraceutical businesses: manufacturers with their own cGMP facility, contract manufacturers running someone else's formula, brand owners and private label sellers whose name is on the label, raw ingredient suppliers, distributors, and marketplace sellers. We ask for your company type, product categories, annual revenue, SKU count, and sales channels, because carriers price ingestible products on exactly that profile.
You will also tell us whether you have third-party COAs and cGMP documentation, whether you carry coverage now, the limits you want, and whether you want general liability, cyber, product recall, workers compensation, or EPLI quoted alongside. If you are not sure about limits, choose "recommend limits" and we will walk you through the trade-offs.
Why supplement companies need product liability
General liability rarely covers an ingestible product claim. Allegations of illness, contamination, undeclared allergens, or label and claim disputes typically fall outside a standard GL policy or land in an exclusion. Product liability responds to those claims, including defense costs, which often exceed the amount demanded.
Retailers and marketplaces frequently require a certificate naming them as additional insured with specific limits and wording before they will accept a SKU. If you are launching into a national retailer or a marketplace, confirm the insurance requirements early, since they can drive both the limit you buy and how the policy is endorsed.
What underwriters ask for
- cGMP compliance documentation, whether from your facility or your contract manufacturer
- Certificate of analysis (COA) practice, including identity, potency, and contaminant testing
- Label review process: structure/function claims, supplement facts, allergen and Prop 65 statements
- Recall and adverse event procedures, plus any history of either
- Revenue by product category and by sales channel, since stimulants, weight management, and certain botanicals price differently
Gathering those items before you apply tends to shorten the underwriting turnaround and can affect the terms you are offered. Premiums vary widely by product mix, revenue, and limit, and any figure we discuss is indicative and subject to underwriting review.
What happens after you submit

Submitting this form does not bind coverage. An application and carrier underwriting review are required before any policy is issued. Prefer to talk it through? Call (888) 998-7742. CA License #0G81238.
